Preparing for Birth: What You Need to Know About Perineal Massage

Birth preparation is about more than packing the hospital bag. One of the most impactful things you can do in the final weeks of pregnancy is prepare your pelvic floor—and that includes learning about perineal massage.

What Is Perineal Massage?

Perineal massage involves using gentle, targeted pressure to stretch the perineal tissues—the area between the vagina and the anus—during late pregnancy. It’s typically started from 34 weeks onwards, and can be performed by the woman herself, with a partner, or with assistance from a trained women’s health physiotherapist.

While it may sound a little unfamiliar or awkward at first, many women find that with guidance, it becomes a powerful part of their birth preparation.

What Does the Research Say?

Perineal massage has been extensively studied, and the evidence is clear. A 2020 systematic review and meta-analysis by Duarte et al.¹ reported that perineal massage, when practised from 34 weeks gestation, was associated with:

  • 21% reduced risk of episiotomy
  • 64% reduced risk of third- and fourth-degree tears
  • Reduced length of second stage labour
  • 70% reduction in postnatal anal incontinence
  • Decreased postnatal perineal pain

These are significant outcomes for both birth experience and long-term pelvic health.

What to Expect in a Session

  • Explain how often to practise, and how to adapt and progress it to your current pelvic floor tone
  • Offer in-clinic support (1–2 sessions per week for those who need assistance)
  • Teach partner-assisted perineal massage if this is something you’re comfortable exploring
